Immunizations

The following chart outlines required immunizations for students in Texas schools. By law, a student's immunizations must be current or in-series or the student must have a notarized Conscientious Objection form on file (see below). A student's immunizations must be up to date regardless of participation in the Virtual Academy or Face to Face instruction. Questions -- please contact me!

Exemption from Immunizations for Reasons of Conscience

Parents may request the above exemption form from the State of Texas.

  • Form must be completed and signed by parent/guardian

  • Form must be notarized

  • Form is good for two years from date of notarization

  • Forms will be mailed from Austin, TX, to parent/guardian; the student will be excluded from school until immunizations are current or a current exemption form is presented to the school (original copy).

  • Further information can be obtained from the Texas Department of State Health Services

  • NOTE: if the school experiences an outbreak of any of the diseases covered by immunizations, any student not fully immunized may be excluded from school for the health and safety of the student as well as other students.
