Enrollment forms are available in the main office at CJH and at the district office. When enrolling your student please have the following items:
Proof of Residence (Required -current lease agreement with listed occupants OR mortgage statement dated within the last 30 days, )
Immunization records (Required - students will not be able to start school until immunizations are provided)
Unofficial transcript and/or withdrawal grades from previous school
Copy of I.E.P. (Special Education students only)
Copy of birth certificate
Required records from other schools may be hand carried or faxed at your request. Incomplete paperwork may delay your student's enrollment. Once your student has been officially enrolled we will request your student's records from their previous school.
