2016-17 Enrollment Information

Welcome to Chaparral High School for 2016-2017 school year

Please complete and return to the Chaparral Counseling Office the following required documentation for enrollment. Once these documents have been completed and submitted the counselor will have 24 hours to review the documents. After review the counselor will contact you to schedule an enrollment appointment. The Parent/Guardian is responsible for providing the following documents:

  • Proof of Residence Please submit one of the following for proof of residence:

    • Warranty Deed

    • Lease/ Rental Agreement

    • Deed of Trust

    • Property Tax Notice

    • Purchase Agreement (valid until closing of property, then we will need the Warranty Deed)

    • Notarized Proof of Residency Form (Available in Counseling Office – Use if you are living with another family. The person you are living with must provide their Proof of Residency along with this form.)

  • Birth Certificate

  • Immunization Record

Colorado State Law states that you MUST have the following immunizations at time of enrollment or the student may not be admitted:

    • Required for all students

      • 5 DTaP (Diphtheria – Tetanus – Pertussis)

      • 1 TDAP

      • 4 Polio

      • 2 MMR (Measles-Mumps-Rubella)

      • 3 Hepatitis B

      • 2 Varicella (Chickenpox – Vaccinations or History of Disease)

  • Unofficial Transcript / Report Card from previous school

    • Counselors must have at time of enrollment to help with credit check and scheduling classes.

    • Report Card or Unofficial Transcript for incoming 9th graders.

  • Withdrawal from 2016-2017 school of attendance

    • State law states that your student may not be enrolled at two schools at the same time.

    • Withdrawal paperwork must include transfer grades for current courses

  • Proof of Guardianship (if applicable)

    • Temporary guardianship papers must be completed with the Douglas County School District Student Data and Information Services department before we can enroll your student. Contact them at 720-433-0095.

  • Special Needs (if applicable): Please provide a current copy of an IEP – Individual Education Plan, 504 Plan, ALP – Advanced Leaner Plan, or ELL – English Language Learner documentation.

  • Completed Chaparral High School Enrollment Forms and Course Selection Sheet

  • Express Check-In: Must be completed online within one week of your counselor appointment. Instructions will be given to you at the enrollment appointment.
