Here you will find all the important forms and links that will be needed throughout your senior year. Mrs. Cleghorn will strictly use SENIOR GOOGLE CLASSROOM to send out important Senior Updates, Dates, College Visits, and Scholarship Information. It is IMPERATIVE that ALL seniors subscribe to get this information. Parents, Guardians, Families, we welcome you to subscribe to this as well to stay informed!

Program of Studies: SENIORS should be utilizing the Program of Studies that you ENTERED 9th grade with to determine Graduation Requirements, Class Rank and Etc. To access the 2022-2023 Program of Studies which you entered in with will provide you with Graduation information including Class Rank, GPA, and etc., Click here!
Summary of Graduation Requirements: Click here for Graduation Requirements for the Class of 2026. Mrs. Cleghorn and Mrs. Saxton will begin meeting with Seniors individually to go over each senior's standing for Graduation.
Required State Diploma Seal Descriptions -- Students in the class of 2025 must also earn State Diploma Seals for Graduation. To meet this requirement, you must earn 2 State Diploma Seals or 1 State Diploma Seal and 1 Local Diploma Seal. Click here to access an outline of the available State Diploma Seals for Graduation.
Required Local Diploma Seals -- Students in the class of 2025 can also earn a Local Diploma Seal for graduation to meet the diploma seal graduation requirement (2 State Diploma Seals or 1 State Diploma Seal and 1 Local Diploma Seal) in the following areas Community Service Seal , Fine Arts Seal , Student Engagement Seal
Transcript Request Form: Transcript request forms MUST be completed for each College/University/Trade School you apply to and turned into Mrs. Cleghorn in order for your transcript(s) to be generated and sent to Colleges/Universities/Trade School. This should be completed for each College/University/Trade School and turned into Mrs. Cleghorn AFTER you complete the application(s). Hard copies are also located in the College and Career Corner in the Main Office.
College Visit Form: College Visit Forms should be completed when you schedule a visit on Campus. Please print/pick up in the Main Office College and Career Corner and complete and return to Mrs. Ulam when you return from your visit. Senior Brag Sheet: ALL Seniors will be required to complete a Senior Brag Sheet. This will be used to write Letter's of Recommendation and etc. For a printable version of the Senior Brag Sheet click here!
CHS Common Application for Local Scholarships: The CHS Common Application will be used for several local scholarships that will be available to our graduating seniors. This is a required document that must be completed by ALL Seniors. Mrs. Cleghorn will not accept late applications!! Seniors will be required to complete this application in the CHS Guidance Senior Class of 2026 Google Classroom! A due date will be posted when it is assigned.
Best Colleges in Ohio: Seniors as you begin and/or continue the College Search, you may have questions about the Universities/Colleges you are looking in to Best Colleges in Ohio can assist you with this.
College Rep visits will be scheduled throughout the Fall and Spring. When dates are set, sign ups will be available in the Library.
FAFSA -- Free Application for Federal Student Aid is required to be completed by all seniors. All seniors , regardless of plans to attend college/university in the Fall SHOULD complete the FAFSA. If you need assistance completing the FAFSA, click here. A Mount Union Financial Aid Advisor will be coming to CHS to present to our Senior class about the FAFSA. Once this is scheduled, we will send out date on the Remind App and Clipper App!
FAFSA Presentation: A presentation on the FAFSA will be scheduled in the Fall. The University of Mount Union offers a complete overview of the FAFSA and will also provide a FAFSA Completion night. When these events are scheduled, they will be posted here!
The Common Application -- To access the common application click here. You can search your list of schools and complete the common application to eliminate having to complete several college applications if the schools you are applying to are listed on the Common App. Please keep in mind that there will still be application fees for the schools you apply to.
ACT Supercourse Info -- If you are interested in an ACT Course to prepare for an upcoming ACT test. Tuition for the course is determined by enrollment. This super course includes the Official ACT Prep Guide with 100% actual questions and tests. Class time is test-taking instruction and NOT for practicing. Motivated students who attend every class and completes all assignments regularly increase their scores by 2, 3 or more points! A flyer will be attached here when dates are available!
