We want to get back on track with being a high-performing team that shows up on time and this form will support us in accountability. Please fill out this form when you notice:

• A team member performing above what’s asked (e.g. a customer gives a positive review, or you notice someone stepping it up)

• A team member is having a performance issue (e.g. a team member shows up late to shift, or a co-worker is having a performance issue you’d like leadership to be aware of)
