Issue: GBD will report the incidence of mortality and burden for a chosen set of external causes. This discussion focuses on identifying the listing of external causes while accounting for (1) policy relevance; and (2) the availability of specific ICD codes for these categories so that estimation is possible. The starting point of this discussion was the categories proposed in the GBD operations manual. This list was reviewed by the expert group and went through several iterations before the final version was proposed as documented on this page.

  • July 1st 2008: Compiled responses with summary recommendations sent to GBD central team and circulated to expert group members for continued discussion.
  • July 25th 2008: Compiled responses recirculated to expert group members for continued discussions.
  • August 21st 2008: Compiled responses updated here for continued discussions.
  • Sept 3rd 2008: Compiled responses with summary recommendations sent to GBD central team along with a briefing note addressed to the core team (see attachment at the bottom of this web page). These are the final recommendations on Discussion 12 being provided by the group to the central GBD team. Expert group members are encouraged to provide further comments until Oct 15 2008, at which point the topic leaders will summarize the discussion topic and prepare for publication.
  • October 17th 2008: This topic is now closed for discussion. Discussion topic leaders are now preparing document for publication.
  • March 5th 2009: Correction - Three ICD10 codes (X71, X77, X78) were listed multiple times in the definition tables - this has been corrected now.
  • April 1st 2009: The external causes definitions proposed in this discussion were carefully reviewed by James and Kavi over the last few weeks. This process revealed some problems that have been corrected. (Resulting ICD10 and ICD9 definitions can be found here). All but one of these were minor issues related with correcting errors, and inconsistencies. The exception was the motor vehicle crash categories, where more substantial issues arose: As the categories are framed in terms of ICD codes, they include both traffic and non-traffic cases, yet the category labels refer to these as road "traffic" injuries. This was recognized as probably misleading. Two solutions were considered: (1) Replace the name of the category with another less restrictive term (2) Redefine the definition to restrict to only traffic deaths. Limited consultation was undertaken, particularly with Lois Fingerhut. Her strong advice was to take the second option for consistency with the corresponding ICE Injury Indicator definition and because of the probably small differences in data scope, at least for deaths. However, after careful consideration of the issue, James and Kavi opted for the first (renaming the category to "road injuries") because of the operational burden of additional work to make the change and other consequences of changing categories at this late stage in the project. However the mortality data obtained for the GBD project will be made available in both specifications.
