Instructions:

1. Download the Personal Data Form
2. Complete the form - the counseling department will give you your cumulative GPA, Class Rank and Number in Class. ACT / SAT scores if needed.
3. We will make copies for you in the Career Center and keep a copy on file for you.
4. Or after completing hit the submit button and Mrs. Pischel in the Career Center will distribute per your instructions.
