All YMCA Programs areas will create a survey for parents/guardians and/or members/participants. The survey will be offered at least annually by all program participants.
To ensure prevention and detection of abuse, program surveys will include the following questions (keeping in mind age and developmentally appropriate language):

Do you feel there is someone at the Y/in the Y program you could go to for help if you are being abused or bullied?
Is there anything else you would like to say about abuse or bullying within the Y/Y program?
Are some areas of Y/Y program unsafe? If so, where?
Have Y employees, volunteers, or other members/participants ever made you feel uncomfortable? If so, please explain.
Are there any circumstances when you are able to be alone with other Y members/participants without Y employees/volunteers present? If so, when?
Are there any circumstances when you can be alone with only one Y employee/volunteer without any other Y employees/volunteers present? If so, when?
Have Y employees/volunteers ever tried to inappropriately contact you via text message, phone call, email, or through a social networking site? If so, please explain.
We are constantly striving to improve at the Y. Do you have any suggestions for what we can do to make our Y/Y program better?
Are you satisfied with the care your child/youth is receiving here? What can we do to make it better?
Does your child/youth ever say anything about his or her (title of employees and volunteers)?
Have employees and volunteers ever inappropriately contacted you or your child/youth about anything other than the program?
Do you ever have a chance to observe your child/youth at the program?
What does your child/youth say about the time he/she spends here?
After each survey collection time is completed, the program director (in conjunction with their Executive Director) will review the program survey results and share the outcomes with the VP or People and Culture. The VP of People and Culture will then determine if any additional action steps are needed to ensure compliance and providing the safest environment possible to the youth that the YMCA serves.
