PEIA's Weight Management Program has a goal to approach weight loss differently consisting of individualized services from exercise and nutrition professionals, personal training, access to a participating fitness facility, baseline assessments with periodic follow-ups, phone coaching from professional counselors, and up to two years of services so long as you demonstrate progress.
This program is not for everybody, but can be beneficial if you qualify and apply. "It takes discipline, support, and a willingness to change your lifestyle for good. Even a modest weight loss of 10% can lower your risk for heart disease, diabetes and high blood pressure. It can make activities of daily living easier like walking, tying your shoe laces, carrying the groceries, and playing with the kids. It can also help to improve sleep, lessen joint pain and alleviate depression."
Application Qualifications:
have PEIA Preferred Provider Plan coverage;
have a Body Mass Index of 25 or greater; or
have a waist measurement of 35 inches or greater (for women) or 40 inches or greater (for men);
be ready to make permanent lifestyle changes and have the approval of your physician or nurse practitioner.

The description below is from their website explaining the process and benefits of the Diabetes Management Program.
"How It Works
If you or your dependents are in PEIA's Preferred Provider Benefit Plan or are a non-Medicare retiree and have diabetes, you can sign up to participate in Face To Face Diabetes Program.
You will select a participating provider in your area. By signing the Participant Agreement form, you are stating that you agree to make regular visits to your provider for counseling services about your diabetes. You will also be expected to follow through on your agreed plan of care.
Saving Money
PEIA will waive co-payments for some diabetes-related prescriptions and supplies if you participate in the Program and keep your appointments while following your plan of care. This could save you hundreds of dollars.
Second Attempt
Members who either failed to comply or dropped out of the program may re-enroll after a 12-month waiting period, which begins on the date PEIA disenrolls you from the program. Second attempt participants will be given a maximum time limit of one year."
