What is an "Official" Transcript?
Employers, colleges, and other higher education facilities usually request official transcripts. An official transcript is signed and stamped by an authorized Colonia High School staff member and mailed in an envelope that is sealed to assure the recipient of its authenticity. A transcript is considered unofficial unless it is signed and stamped and delivered in a signed, sealed envelope.

If you are a Colonia High School graduate, you can request transcripts in person, by mail, or by fax. Unfortunately, we can not give you a new diploma. We do not reprint diplomas but your transcript will have your graduation date on it. It is recommended that you use the form below. Any missing information could delay the processing of your request.
The following information is required:
Signed and Dated Consent/Request Form
Signed valid Photo ID
Year of Graduation or Withdrawal
Date of Birth
Student's Name at time of graduation and current name
Student's current contact information
Name and Address where Official Transcript is to be mailed *
In Person, CHS Graduates must:
Complete the Transcript Request Form
Bring a passport or state-issued photo ID to prove your identity - please call ahead for office hours
By Mail, Fax or Email, CHS Graduates must:
Complete the Transcript Request Form
Include a copy of your state-issued photo ID with Signature to prove your identity

***REQUESTS FOR TRANSCRIPTS MUST BE SUBMITTED BY THE STUDENT IN WRITING. A MINIMUM OF 10 WORKING DAYS PRIOR TO THE APPLICATION DEADLINE.***
In accordance with federal law, academic records are confidential and may be released only with the written consent of the student or parent/guardian if the student is under 18 years of age.
*Please note, to be "Official", transcript must be mailed from Colonia High School directly an institution of higher learning or to an employer. Transcripts mailed to your home address will be marked "Unofficial"
