Task 1: Re-measure the Data
Now it is time to remeasure your post-intervention data using the same method used for the baseline data to determine the effectiveness of improving the health care system you are addressing
Re-measure timing: When re-measuring your post-intervention data, consider waiting for a period of time to see the "steady state" of the intervention (e.g. Allow your intervention to be in place for 1 month before remeasuring). Also, consider if the time of the year that the measurement is done matters. For example, influenza rates in June should not be compared to November as your data may be influenced by higher vaccination rates in Fall.
Data display after re-measure: Use the same charts created in Module 3 to show baseline and intervention data. This will graphically illustrate the impact of your intervention on the health care system.
Take a moment to reflect on the impact of your problem
Did your project make a difference in the care provided to patients in this health care system?
Did you address the intended problem?
Did the measure you chose adequately assess this problem?
Continuous improvement: Although your time in fellowship is brief (1-3 years), the QI cycle should be ongoing.
If you were to be able to continue this project, the next step would be to re-evaluate your project's impact to see if further improvements can be made to the system. You might choose to further improve your initially chosen measure or apply one of the other measures you had brainstormed.
Is there a way that you can pass this project along to another fellow, staff member, or someone else in the division to keep it going after you are gone?
