Task 1A: Root Cause Analysis in Healthcare Systems
View Presentation: Process Maps and Cause/Effect Diagrams:
Extra Reading (if interested): Systematic Root Cause Analysis of Adverse Drug Events in a Tertiary Referral Hospital (Rex, et. al. 2000).

Now that you are familiar with various approaches to root cause analysis you will analyze your problem to arrive at better clarity.
Task 1B: Map the Problem (Process Map)
Create: a process map for your identified problem:
You may choose to create the process map by hand, which is acceptable.
Alternatively, create the process map using this drawing tool: Draw.io

Save and Export: the completed process map document. You will upload the diagram in Task 4.
Use one of the tools below to create a map that illustrates the steps that occur around the process you are investigating.
Various shapes will represent events or decision points along the path to the ultimate outcome.
This figure will include the people involved in the process and all the steps needed for the process to be carried out.
If using Draw.io: Save the diagram for later editing:
Select File -> Save.
This creates a .xml file in the downloads location of your computer that can be opened later.
Export the diagram to submit for feedback:
Select File -> Export as.
Choose Image or PDF.
Note the location of where the file was saved.
Upload the saved file when you get to Task 4 below.
Brainstorm: Complete the diagram for your proposed problem.

Save: the completed process map. You will upload it at the end of this module.

Tip: Once you've downloaded the diagram, open it in Adobe Reader.
Other applications will not allow you to edit and save the diagram.
Do not use the browser preview, it is for viewing only.
Now that you have mapped where a problem may exist within a delivery system you will identify contributing factors to the problem using a fishbone diagram.
Task 1C: Identify Contributors to the Proposed Problem (Fishbone Diagram)
Download and Save: a blank fishbone diagram (PDF document).
Task 2: Measuring the Quality Gap
Complete: Self-assessment quiz to check your understanding (not graded or submitted).
Task 3: Identify Measures
Brainstorm: Five measures that could be used to assess the Quality Gap in your project. Please be sure that at least 1 idea falls into each of the 3 types of measures: Outcome, Process and Balance Measures.
Select: the measure that is best for your project.
Save: the results of your brainstorming.
Once you have gotten this far you will now submit the products of your work.
