Please complete tasks 1-3 below, then submit your results using the "Submit for Feedback" button at the bottom of the page.
For an overview of the timing of your project refer to: Timeline Checklist
Task 1A: Familiarize yourself with IRB and HIPAA
Complete: IRB self-assessment quiz
Now that you have familiarized yourself with the basics of IRB approval and HIPAA considerations, your next task will be to complete the IRB Decision Tool for your QI project.
Task 1B: Complete IRB Decision Tool
Complete the IRB Decision Tool: by visiting the IRB web page below and clicking on the "Research Decision Tool" link.
This verifies that you are conducting a QI project and not a research project.
If your project is indeed a QI project, you will receive an "IRB Exemption" certificate.
If your project is deemed research, please contact your QI Program mentor and the Health Sciences IRB Office.
Keep the IRB Decision tool certificate that is generated at the end of the questionnaire for own records.
Task 2: Gather your baseline data to determine the quality gap
Now that you have selected your measure and determined that your project is QI and not research, apply your measure to your clinical setting and gather baseline data.
This Baseline Data will verify whether a Quality Gap (your perceived problem) indeed exists. This is the data before the intervention has been put into place.
Remember to gather data for both the numerator (number with or without the problem) and the denominator (number of patients or events screened) to verify the extent of the quality gap.
This may take you some time to complete, so remember to account for this in your project timeline; Plan a 1-3 month window for completion.
Be sure to save this information, you will submit at the end of the module
