Minor Incident Report Form - Please fill in when a student has injured themselves during a lesson and might perhaps require first aid treatment.

Concern form - Please make a copy, edit,  and email to HOD.  Once approved, email copy to secretary and cc HOD and HOY.

Steps to take before issuing a concern form:

• Warning given to student. 

• Send student to the HOD.

• If concerns persist, concern form is issued. 

• Log all behaviour events un Go$ school tagging HOD and HOY
