What is Microsoft Forms?

Microsoft Forms is an application that lets users create surveys, quizzes, polls, and other types of forms. In addition to tools offered by Canvas for Quizzes, it could be used by teachers, and other staff to collect  feedback from students, parents, or other staff.  

How do I sign in?

Similar to other Office365 products (Outlook, Word, etc.) you can sign in to Microsoft Forms utilizing your West Allegheny credentials.

Creating a Microsoft Form Basics

Start a New Form

1) Navigate to the Microsoft Forms page with your West Allegheny credentials. Please note: you can also go to office.com, look for the Microsoft 365 icons on the left, and then select "Forms"

2) Select New Form

3) Title your form and give a description (optional)

Add questions

1) Select +Add new or +Add Question Now to add a new question to your form.

2) Choose your question type (Examples: Choice, text, rating, date, etc.)

3) After you have added questions, you can select preview or click the eye icon to see how your form will look.

4) Enter sample answers into the forms and select submit.



Send a form and collect responses

1)  Open the form and navigate to Collect Responses
2) Under Send and collect responses, select the audience you would like to see the forms. Your options are Anyone can respond, Only people in my organzation can respond (anyone with a @westasd.org e-mail) or Specific people in my organization can respond.

3) Select Copy to Copy the URL

4) Select an invitation option (Send to a Contact, QR Code, or Embed code)

Checking your Form Results

Forms Summary

Open the form for which you want to review the results, and then select the Responses tab. You will have access to see summary data about your form, such as responses, time it took to respond, and a chart that breaks down the responses.






Viewing details for each question

Select View Results to see individual details for each response.

Select the More Details link under individual questions on the Responses tab to see more details for that question.
