Course/Level Change Form - to be used when requesting a change of schedule. Form should be dropped at your House Office/School Counselor. Please be sure to complete both sides of the form. Students are to remain in their current class until notified their request has been facilitated.
Independent Study Form - to be used when requesting to complete an Independent Study Project.
Student Assistant Application - please request at house office.
Senior Grade "Hold Form" - emailed at the end of quarter 1 and 2.
Course Selection Form to be used when planning out your course of study.
Course Level Change Form must be printed, signed by those indicated on the form and returned to your counselor.

The following are required for graduation from Wellesley High School:
Earn 136 credits* in grades 9-12, Pass 4 years of English, 2 years of math (3 years beginning with the Class of 2028), 2 years of science, 2 and ½ years of social studies (1 of which must be U.S. History), 1 quarter of Physical Education in 9th, 11th and 12th grade, and a semester of Health in 10th grade.

In Massachusetts, public school students must meet both local and state requirements in order to earn a high school diploma. The Massachusetts Education Reform Law of 1993, G.L. c. 69, § 1D, requires that all students meet the Competency Determination (CD) standard, which is usually done by earning a passing score on MCAS (until December 2024). Students scoring in a certain range, designated by year of graduation, may require additional courses beyond the WHS graduation requirements to meet state graduation requirements, including, but not limited to taking 4 years of math and academic support courses.
