Before being able to participate in a sport, you must complete your DragonFly and "Ticket-to-Play". There are several steps to completing these, and it can be a bit overwhelming, so this page is dedicated to help you with any questions you may have. The Wake Forest's Sports Medicine website is an excellent resource and contains all the information you need to complete your DragonFly and your Ticket-to-Play.
DragonFly Account
Before you apply for your "Ticket-to-Play" (which is just a Google Form you fill out..linked at the very bottom of this page) you MUST create/complete your Dragonfly account. In order to get a 100% for your DragonFly account you need to...
Create a Dragonfly Max account (if you don't already have one) . If you already have an account, please do not create a new one! If you have forgotten your password or username, please follow the site directions to find this information.
If you are a rising freshman who already has a Dragonfly account, here is a resource to help you transfer your account for high school.
If you have done dragonfly for another school (or middle school): Log into your account and in your profile in the upper right corner, there should be a button that says "+ connect to school or organization" click that and search for Wake Forest High School.
2. Upload Physical and Medical Eligibility forms.

Here are individual copies of each form. You will need to print them then upload them into your DragonFly Max account.
Need a physical?! OrthoNC does physicals for $10 and they accept walk ins.
3. Complete any team rules/documents within Dragonfly.
*BE CAREFUL. There is a form which asks which sport(s) you DO NOT give your child permission to participate in! It is an odd question and most people select the sport(s) they want to participate in; which makes you ineligible to participate in those sports....until you go back and fix the form. So be aware and try to fill it out correctly the first time.
Ticket-to-Play
Once you have completed your Dragonfly account, you will need to complete your "Ticket-to-Play". The ticket link is only open for a certain window so please make sure you complete it within the time frame (dates listed once you click the button).
