If you are required to complete a CORI form for any reason you must complete both sides. You must appear in person to submit the CORI form and provide a copy of a government issued photo i.d. such as a driver's license or passport.
If you are unable to appear in person then you may have the form notarized by an authorized Notary Public and mail the original form to Walpole Public Schools, 135 School Street, Walpole, MA 02081, ATTN: Human Resources.
