ALL EXSERVICEMEN IN MAHARASHTRA ARE HEREBY ADVISED TO REGISTER THEMSELVES ON TWO WEBSITES 

• First with www.ksb.gov.in and then further apply for issue of ZSWO Identity Card

  • Prerequisites

    • Scan of passport photo in civilian clothes

    • Scan of Complete Discharge/Service Book in PDF

    • Scan of PPO showing name of family pensioner

    • Scan of a Address proof - UID/Electric or Gas bill

    • In case you are original domicile -please upload Birth Certificate or Xth or XIIth Marksheet

• Second with www.mahasainik.maharashtra.gov.in- This is compulsory, failing which issue of I card will not be possible

    • Scan of ECHS Card if held

    • In case you are original domicile -please upload Birth Certificate or Xth or XIIth Marksheet

1. Government of India has a DIGILOCKER which is a good site for storage of personal document scans. Please upload all your important document scans to the digilocker site. Primarily Every ESM should have a copy of 

• SERVICE BOOK/DISCHARGE BOOK/DISCHARGE Certificate

• Pension Payment Order - Original and Corrigendum/ E PPO

• Copy of their ZSWO I Card

• Copy of their CSD and ECHS Cards     placed on the Digilocker

2. As per instructions of Kendriya Sainik Board (KSB) a Zilla Sainik Welfare Office Identity card is issued to 

• An Exserviceman who fits in the definition of an Ex-servicemen(ESM)

• A Widow of an ESM

There are five varieties of ESM Cards provided by KSB for issue to ESM:

• ESM I Card

• ESM Retired Officers Card

• ESM Widow Card

• ESM War Widow Card

• ESM War Disabled Card

3. To Obtain a ZSWO I-Card, ESM should first register with the website - www.ksb.gov.in

4. During the registration process care should be taken to select the correct RSB and ZSB where the ESM is most likely to settle. Once the registration process is complete the ESM needs to complete the ICARD application process and upload relevant documents, namely Discharge/Service Book, PPO, Address Proof, NOC<where change of district is needed> as applicable. (It is important the scanned documents are of a limited size. It is suggested to use https://tools.pdf24.org/  if you need to reduce the size of the scans. Before Finally submitting the form, please check that the concerned PLACE shows the correct target District name and not any other district.

5. On completing this process, the ESM should visit the concerned ZSWO Office along with two Stamp Sized photographs and copies of the documents uploaded on the website. Please remember to carry the ORIGINAL Discharge Book/Service Book, PPO while visiting the office as an endorsement in regards issue of I-Card needs to be completed on the original documents.

REISSUE OF IDENTITY CARD

Reissue of Identity Card is done under the following circumstances and as per KSB guidelines:

1. Loss of I-Card - Primary activity to be done by ESM is to lodge a Police FIR at the earliest. Wait for one month as cooling off period in case the said I-Card is retrieved by the Police or found by you. After on month revisit the Police Station and obtain a stamp from the PS stating that item has not been found. Then apply for a Fresh I card as per procedure enumerated above.

2. Mutilation of the I Card - ESM to produce the mutilated I card at ZSWO Office and before that follow the procedure for I card application as enumerated above.

3. Change of Particulars - In case of change in rank eg: Hony Rank or in case of change of address within the District. However, these cases will be dealt with on a case to case basis.

ISSUE OF NOC TO APPLICANTS WITHIN THE STATE OF MAHARASHTRA

Email application from applicants will be accepted. Please obtain office email id from Department Of Sainik Welfare page.

PROCEDURE TO BE FOLLOWED BY APPLICANT:

1.  In case the applicant is a first-time applicant he/she should apply to the concerned ZSWO (district mentioned in address column of Discharge Book or Service Book) by email providing copies of certain documents. Applicant should mention that he has not drawn an I-Card till date and now wishes to obtain an I-Card from the target district. The ZSWO will issue an NOC to the target district mentioning that an endorsement in regards the NOC be made by the  I-Card issuing office in the ESMs Discharge Book.

Documents required to be attached: Copy of Service Booklet complete with details of last recorded address.

2.  In case the applicant holds an I card from a particular district he/she should apply to the concerned ZSWO for an NOC to the target district on email by providing a copy of the I-card held; with assurance that the said I-card will be deposited to the target District while applying for a fresh I card in the Target District.

Documents required to be attached: Copy of Issued I-Card & copy of Service Booklet complete with details of last recorded address. In case of loss of old I card, a copy of the FIR made(dated 30 days prior to application)

PROCEDURE TO BE FOLLOWED AT ZSWO (Issuing NOC):

1.  On receipt of email on the official email, the concerned ZSWO should verify the correctness of the application and issue an NOC by email to the target District mentioning whether old I-card needs to be deposited by the Target District. The NOC should also mention whether there is need for endorsement of the issue of NOC has been made or needs to be made in the Discharge Book of the applicant.

         Documents to be checked:

·         I Card if it is issued from the ZSWO Office

·         Details of applicant as per the Office Software

·         Discharge Book in case of first-time applicant.

Please note that address proof check here is not a requirement and hence there is no need to ask applicant ESM to provide any kind of address proof or such document.

2.  ZSWO Office should email the NOC to the applicant and the Target District using official email id only.

PROCEDURE TO BE FOLLOWED AT ZSWO (Target District for NOC):

1.  Ask applicant to apply for I card as per prevalent system in place (Manual or through KSB website)

2.  Check details of NOC and need for endorsements as per the NOC.

3.  Issue I- card to ESM and ensure necessary endorsements as per regulations are made clearly specifying dates.
