In accordance with the Healthy North Carolina 2030 mission, each local health department is responsible for conducting a Community Health Assessment (CHAs) and developing Community Health Improvement Plans (CHIPs) with community partners. Progress is reported in between CHA reports through State of the County Health Reports (SOTCHs). The "CHA-CHIP-SOTCH" cycle occurs every 3 years. These reports give us an overview of the major issues in our county, and the opportunity to develop programs and solutions that make a measurable difference.
