We provide two case management programs to help you and your family stay healthy and supported. Our team is here to walk beside you every step of the way, connecting you to the right care and resources for your needs.

Care Management for at Risk Children
(CMARC)
CMARC is a free program for children from birth to five years of age that recieve medicaid. A care manager is available to discuss concerns and their child's individual needs. Our social worker is specially trained to listen and assist with finding the right help
Services include:
One on one counseling
Service referrals
Attending doctor visits
Child Development Education
Assistance with developing parenting skills
Care Management for High-Risk Pregnancies (CMHRP)
Care Management services are available to women who enrolled in NC Medicaid and are pregnant and or in the postpartum period. Our social workers provide one on one support for your during this time.
Services include:
One on one counseling
Service referrals
Attending doctor visits
Care coordination
General support during pregnancy and after your baby is born
