Registration Instructions

Registration for a training only occurs through the Parent Portal*: Parents will be required to do a one-time “enrollment” into the Parent Portal. This process will include having your cadet’s ID card on hand as it is needed to link your household. Currently only the “Primary Parent” is permitted to apply for a cadet training. The Primary Parent will then fill out all of the medical history, acknowledgment, and medication forms as they would in the paper file. Once completed, parents will need to acknowledge when applying for trainings, and no longer utilize the paper forms. This will also allow you to quickly update medical information quickly and efficiently for the home unit and training evolution. Once these forms have been completed, you will be able to locate the desired training and “Apply.” If your cadet meets all of the basic requirements, your application will move up to the unit staff for registration.

*Families without internet access can be assisted with the application process by their unit. Please contact your chain of command.

REQUIRED FORMS AND INFORMATION

  1. Go to Quarterdeck under 'Find Trainings'. Register for this training listed under 'Navy Shore: RC-MD-2201)

  2. Complete and submit Report of Medical History. [NSCADM001 pages 3-4] (upload in parent portal or in Magellan)

  3. Complete and submit Medical History Supplemental and Request For Accomodation [NSCADM001, pages 7-10], if applicable upload in parent portal or in Magellan)

  4. For those flying to the training: Email a copy of your airline receipt to our Financial Officer, INST Debbie Hirth: debbiehirth@gmail.com Upon graduation from training, you will be eligible for $400 financial aid from NHQ to help you get to our training. ***You are also eligible for one paid checked bag, round trip, per cadet. Keep and send this receipt to the above.

  5. Complete and sign the COVID-19 Acknowledgement & Liability Waiver (click here) and make sure it is in yout travel service jacket

  6. Complete the REQUIRED Supplemental Information Form(click here)

  7. Pay yout training fee: The primary contact listed in Magellan will receive an invoice from our financial officer when you are confirmed for training whereby you may pay with credit card.

  8. Complete, sign and submit the Parade Agreement of Release (click here) to LTJG Dyer at dlgdyer@gmail.com by June 05.

NO cadet will be confirmed until

all the above is completed and submitted.
