Surgical Implant Consult Protocol (Revised)
Implants are assigned to one of three different clinics=
Oral and Maxillofacial Surgery
Graduate Periodontics
Graduate Prosthodontics
The process of implant patient/surgical clinic assignment and follow-up treatment will be as follows:
Student presents with a pre-doctoral implant case
Student schedules a PRETP appointment in advanced prosthodontics for impressions, facebow and gathering of preliminary data
Student performs a wax-up of the area to receive the implant(s) as the first step in fabricating the radiographic/surgical stent
Student schedules/conducts the ATP in the Treatment Planning Clinic
While in TPC, the student will complete an Internal Referral form, using the new Tab (Implant Consult)
CBCT order entered on this form
Form approved by the covering Advanced Prosthodontics faculty
Student will request Surgical Implant Consultation from GPA Veronica Guzman
Tell Veronica:
Student name
Number/chart number
implant sites, etc
Veronica assigns case to the specific graduate clinic.
Surgery clinic can't be changed
Veronica sends the Internal Referral Form by Running Man to BOTH Radiology (RAD) and the Admin/Referral message group for the selected clinic, as follows:
RAD - Radiology – (Admin User: Rio Quintero-Martinez)
ADVREF –Advanced Prosthodontics – (Admin User: Veronica Reynoso)
OSRE –Oral Surgery – (Admin User: Barbara Nichols)
PERIOR – Grad Periodontics – (Admin User: Justine Sunga)
Student fabricates the radiographic/surgical stent.
Highly preferable: have the student schedule the CBCT appointment
Patient can schedule IF approved CBCT order has been placed and sent to Radiology.
Rio, in Radiology, will then send the CBCT to the selected clinic.
Student follows up selected clinic to request a pre-doctoral implant surgery consultation
Will receive instructions as to how to follow through with case execution and follow-up
Student MUST accompany patient to=
Surgical implant consult appointment
Surgical placement appointment
Surgical clinic should complete the bottom of the Internal Referral form used above:
They record the pertinent implant(s) identification information (company/size/lot/etc)
Then, send the form via running man back to the dental student
PROSTHODONTIC / RESTORATIVE PHASE:
Faculty confirms "Implant Ready to Restore" (tab 3) signed off by the surgical clinic
Faculty will approve the impression copings, the analogs and the abutments needed

Students will assign this form to Advanced Prosthodontics Admin (Guille) via running man
Components have arrived (~2 weeks), they can be picked up from the Advanced Prosthodontics Division Office during the designated pick-up times
