Charles OBrien now retired from the RIC with his wife Belinda and some of their children, Notice how the 4 eldest still living with them were born in Sligo and the youngest 2 were born in Mayo. Mary the eldest would die several months later  from a burst appendix. Belinda ( Bella) is not listed as she was living in Carrownaboll with Annie & Pat. Also note that Charles & Belinda had lost another child, the form says Belinda had 8 children with 7 surviving. The deceased child was named Charles, born in 1909 and living just 12 days.
