Postsecondary Transition Plan (PTP)
A Postsecondary Transition Plan (PTP) is a structured document, required for the age of 14+ in Wisconsin but could be used earlier, developed for students with disabilities as they prepare to transition from high school to adulthood. It outlines the student's goals and objectives related to education, employment, and independent living skills. The PTP is designed to guide the implementation of services, supports, and activities that will help the student achieve their desired outcomes and successfully transition into postsecondary education, vocational training, or employment opportunities.
A well developed and implemented PTP is essential for incarcerated youth with disabilities as it lays the groundwork for their future success by addressing the youths educational, vocational, and independent living needs both during and after incarceration.
Pre-Employment Transition Services (Pre-ETS)
Pre-Employment Transition Services (Pre-ETS), which are a componenet of the PTP, are services designed to help students with disabilities, including incarcerated youth, prepare for employment or postsecondary education and are should be utilized to reach any postsecondary goals. They include activities such as career exploration, work-based learning experiences, counseling on postsecondary opportunities, workplace readiness training, and instruction in self-advocacy.
Pre-ETS are important for incarcerated youth because they provide essential skills and knowledge that can increase successful reentry into their home, school, and community. These services help build confidence, develop job readiness skills, and provide opportunities to explore career paths. Providing incarcerated youth with these tools and resources early on reduces the likelihood of recidivism.
Curriculums
Pathful Explore assists students in finding careers and educational paths that match their interests and strengths. It combines interactive tools, assessments, and real-world exploration videos to prepare students for college and/or careers.
Merging Two Worlds (M2W) is a transition curriculum developed specifically for individuals in residential and secure settings by the Arizona Department of Education. The full curriculum consists of 40 lessons that provide instruction in life skills, career development skills, resiliency skills and social skills. The entire curriculum can be downloaded via the website.
The Free WiTransition App which aligns directly with the Postsecondary Transition Plan (PTP) was designed by the Transition Improvement Grant (TIG) to increase self-determination and self-advocacy for all Wisconsin youth in the transition planning process. The app, which is also available in paper copy and in English and Spanish, aims to assist students and their families in the transition planning process, emphasizing the student's preferences, interests, strengths, and needs as top priorities.

Transition Tennessee Student Lessons
Transition Tennessee's (TransitionTN) online platform hosts specific lessons focused on going to work after high school. These lessons cover essential topics such as career exploration, job readiness skills, workplace etiquette, and finding employment opportunities.
O*NET Online is sponsored by the U.S Department of Labor, Employment & Training Administration and is a useful tool for students seeking a deeper understanding of what their interests are and what careers may align to those interests.
