New Course Proposal: Use this form to request approval for a new 200 level course offering within GSAS. The department chair and graduate program director must approve this proposal, indicating that the faculty has reviewed and support the change.
Change Existing Course Proposal: Use this form to request a change for an existing 200 level course. The department chair and graduate program director must approve this proposal, indicating that the faculty has reviewed and support the change.
Change Existing Credit Value Proposal: Use this form to request a change for the credit value of an existing 200 level course. The department chair and graduate program director must approve this proposal, indicating that the faculty has reviewed and support the change.
Remove Existing Course Proposal: Use this form to request that an existing course is removed. The department chair and graduate program director must approve this proposal, indicating that the faculty has reviewed and support the change.
New Program Proposal: Use this form to propose a new program (degree, certificate, etc) for approval by the Policy and Programs Committee. Please note that program proposals must receive approval from both your department and the GSAS Dean’s Office before being reviewed by P&P.
New Fifth Year Program Proposal: Use this form to propose a new fifth year program for approval by the Policy and Programs Committee. Please note that new fifth year pathways must receive approval from both your department and the GSAS Dean’s Office before being reviewed by P&P.
Add a Specialization: Use this form to propose a new specialization/new specializations for approval by the Policy and Programs Committee. Please note that new specializations must receive approval from both your department and the GSAS Dean's Office before being reviewed by P&P.
Change in Program Name: Use this form to propose a new program new for approval by the Policy and Programs Committee. Please note that all name changes must receive approval from both your department and the GSAS Dean's Office before being reviewed by P&P.
Change in Program and/or Credit Requirements: Use this form to propose updated program requirements and/or a change in credit requirements for approval by the Policy and Programs Committee. Please note that all program/credit changes must receive approval from both your department and the GSAS Dean's Office before being reviewed by P&P.
Change in Program Modality: Use this form to propose a program modality change for approval by the Policy and Programs Committee. Please note all modality changes must receive approval from both your department and the GSAS Dean's Office before being reviewed by P&P.
Notification of Program Licensure/Accreditation Change: Use this form to notify the Policy & Programs Committee of an approved change in program licensure or accreditation. Please note that all changes to licensure or accreditation should be coordinated with your department and the GSAS Dean's Office before notifying P&P.
