New Course Proposal: Use this form to request approval for a new 200 level course offering within GSAS. The department chair and graduate program director must approve this proposal, indicating that the faculty has reviewed and support the change.
Change Existing Course Proposal: Use this form to request a change for an existing 200 level course. The department chair and graduate program director must approve this proposal, indicating that the faculty has reviewed and support the change.
Change Existing Credit Value Proposal: Use this form to request a change for the credit value of an existing 200 level course. The department chair and graduate program director must approve this proposal, indicating that the faculty has reviewed and support the change.
Remove Existing Course Proposal: Use this form to request that an existing course is removed. The department chair and graduate program director must approve this proposal, indicating that the faculty has reviewed and support the change.
Change an Existing Program Proposal: Use this form to request a change for an existing graduate program. The department chair and graduate program director must approve this proposal, indicating that the faculty has reviewed and support the change.
New Program Proposal: Use this form to submit a new program (degree, certificate, etc) for approval by the Policy and Programs Committee. Please note that to submit a program for approval you should already have approval from both your department and the GSAS Dean’s Office to move forward in the process.
New Fifth Year Program Proposal: Use this form to submit a new fifth year program for approval by the Policy and Programs Committee.
