Goal: Practice has met at least 75% of its targets and sustained improvements in practice-identified metrics for at least one year.

Strategies:

Select a patient-related quality measure (like milestone 13)
Collect and analyze data monthly
Identify process issues needing to be improved
Implement change
Measure again and sustain the gain!

What our enrolled practices are doing:

Utilizing the QPP site to review claims submissions,
Prepare performance charts on quality measures that are updated on a quarterly basis.
