REGISTRATION
Register for the test session - Mail or Deliver Application to SLFSC 90 Lincoln St. Canton NY 13617
If you are dropping off Application, please put the form in SLFSC mailbox on the Costume Closet Door.
Submit required payment via cash, check, or Paypal (@slfsc)
Application and Payment needs to be received no later than February 27, 2026
BEFORE RECORDING YOUR TEST SESSION:
Access the following required forms and print hard copies to bring with you on your recording day. These forms must be signed in person during your recording.
RECORDING YOUR VIDEO
Videos must follow ALL Virtual Test Guidelines. Failure to follow guidelines may result in the test not being judged and no refund.
Reminder you must have a Virtual Test Placard and Hard Copies of Performance Affidavit at the time of the recording.
VIDEO & DOCUMENT SUBMISSION
☐ Name video file: Skater Last Name – Test Level – Test Description
☐ Upload video and completed documents (performance affidavit, name and likeness release, and permission to test (non - club member) to a Google or Dropbox Folder
☐ Email the folder link to stlawrencefigureskatingclub@gmail.com
