Here are the results of my recent visit to the VA Center on Clairmont Ave, Decatur, GA.

1. Almost everyone in the Soleil should be eligible for the Bronze Geriatric Clinic. This is a Geriatric (80+) Specialty and Primary Care Clinic for Veterans who are 80 years or older. Younger Veterans with specific disabilities or medical conditions are also cared for here.

2. You get a dedicated direct telephone number:  (404) 728-7663.  

Toll Free, if that’s important, then you can also dial (800) 944-9726, select dial 1 + extension 7663

- Press: 1 for medications 

2 for Nurse

3 for Social Work

4 for Appointments, Travel or other concerns. 

Once you sign up and are OK'ed (Last Four of your Social Security number, if in the system), you get:

• Patient Aligned Care Team (PACT) assigned to you with:

  • • - Care Provider (Doctor)

    • - Social Worker

    • - Nurse Care Manager

    • - Pharmacist 

So if you are 80 or older, you might consider checking this out!
