Technically the intake form is optional. It is a great way to quickly get information to us about your intentions in getting involved. You will use your information from the Intake Form to Document onto your Public Online Personnel Profile (POPP) which is required. Everything you put onto the PUBLIC Intake Form is Public, so please do NOT add any information you do NOT want to be Public.
We also have the PRIVATE Contact info Form. This Form is for Private information and should be for your personal contact information, and anything else that is private that you want to communicate to us about your involvment.
