Incident Report Guidelines
Reasons to Write an IR:
Alcohol
Drugs
Student Conflict
Harassment
Hospital Transport/Medical
Vandalism/Damage
Noise Complaint
Guest Conduct
Suspect(s) Name(s) and Others Involved:
Include information for all parties involved, including Res Life staff, Campus Safety Staff, roommates, etc.
The most important things to include are correct spelling of everyone’s last name and their ID number.
On the RA N: Drive, there is a folder called “Incident Report Resources” that has a list of Residence Life Staff and Campus Safety Officers with their names and ID numbers.
If a student or guest refuses to provide identification, please call Campus Safety. They can call Naperville Police, if necessary.
When entering Res Life or Campus Safety staff information, always use 451 S Brainard St as the address and office phone numbers (for Res Life) or the Main Office (for Campus Safety, 630-637-5826). Do not use personal cell numbers or apartment location
Incident Details/Narrative:
The first time a student’s name is mentioned in your narrative, please include their first and last name with the last name in all capital letters.
EX: When the RAs entered the room, John DOE was sitting at a desk with a beer.
After the first mention of a person, you should continue to use their last name only – still in all capital letters.
EX: John DOE opened the door. DOE stated that they were “totally not doing anything wrong.” Avoid pronouns (he/him, she/her, etc.) Please be specific and tell a story:
How did you find the incident?
Why did you knock? (what did you hear, quotes, etc.)
Name everyone who was involved.
Were the students compliant? Were they willing to give you their ID, name, or contact information immediately? Etc.
What did they say or do that made you feel a certain way?
Please use descriptive action words. Rather than saying someone was mad, please describe how they acted. Did the student raise their voice? Swear at you? Throw things?
What happened at the end? Was the party dispersed? Did students stay in the room? When did the RAs/Campus Safety/AHD/NPD leave? Was there any sort of resolution that evening?
