1) Search your name in the voters list : http://ceo.karnataka.gov.in/search_2019ph1/vernacularSearch_New.aspx
The site is very slow at day time. Try searching between 8pm to 8am.
2) If your name appears in the search results. Note down Part Number , Serial Number and Voter ID Number as per below pictures

3) If your name shows up, you can collect it from Bangalore One, as per instructions at : https://sites.google.com/view/psnvoteridfaqs/status-of-application#h.p_NDfvtwGORoIT. You can vote in this election by showing a screenshot of the search results, and a government photo ID card like Driving licence, passport, aadhar etc.
We cannot help with re-issuing the cards.
