Process for New application or Movement from another constituency

1. Fill the appropriate form

Do not print on both sides. A part of the second page needs to be torn off during processing.

• First Time Voter - Form6 - PSN Prefilled Form
• Moving within Mahadevpura - Form8A - PSN Prefilled Form
• Moving from another Constituency [ You have a voter ID at a different address anywhere in India ] - Form6 - PSN Prefilled Form

For your convenience, Forms are available at the following places. Kindly choose the correct forms, or it will get rejected.

• the Stationary shop at Tower 17 has soft copies of the forms. You can print it there. The shop timings are 11am to 8pm on weekdays, and 11am to 1pm on Sunday.

2. Attach 2 passport size photos [Not Stamp size]

• Stick one in the space provided
• Staple one to the top of the application.
• If you put any other size, your application might get rejected

3. Attach a copy of your age proof. The below are acceptable for age proof.

• Aadhar Card
• PAN card
• Driving License [Will also serve as Address proof]
• Indian Passport [Will also serve as Address proof]
• If a person in class 10 or more pass, he should give a copy of the marksheet of class 10, if it contains date of birth as a proof of date of birth
• Marksheet of class 8 if it contains date of birth
• Marksheet of class 5 if it contains date of birth
• Birth certificate issued by a Municipal Authorities or district office of the Registrar of Births & Deaths or Baptism certificate
• Birth certificate from the school (Govt./ Recognized) last attended by the applicant or any other recognized educational institution

4. Attach a copy of Address proof. the below are acceptable for address proof.

• Passport [Will also serve as Age proof]
• Driving License [Will also serve as Age proof]
• Latest Water / BSNL / VDC Telephone / Electricity / Gas Connection Bill for that address, either in the name of the applicant or that of his / her immediate relation like parents etc.
• Bank Statement
• Valid rental agreement - Atleast 6 months old
• Copy of Sale deed - Even 1 day old is accepted. [from page number 1 to the page on which your flat number is mentioned - no need of the entire document]
• Bank / Kisan / Post Office current PassBook
• Income Tax Assessment Order
• Any registered post / letter / mail delivered through Indian Postal Department in the applicant’s name at the address of ordinary residence
• Ration Card

Aadhar card is ***NOT*** accepted as proof of address

5. If you are Moving from another Constituency and have your old Voter's ID card. please attach a copy. It is not mandatory.

6. Final Checks

• Sign all the supporting documents
• Mobile number and e-mail id are required. If there required, election officers will contact you.
• Checklist at the last page of the pre-filled application ***MUST*** be completed.
• Aadhar card is ***NOT*** accepted as a proof of address. It is accepted only for age proof.
• Kindly staple all the pages together. Paper clips / Binder clips are not reliable enough.
• Pls remember, this is a volunteer driven effort. Pls make every effort to submit your application properly. If you have any doubts, pls contact us before submitting.

I just moved in to PSN, can I apply?

If you have an address proof other than rental agreement, then you can apply.

Rental agreements have to be more than 6 months old for it to be accepted.

If owning, your sale deed can be used as address proof immediately. No need to wait before applying.

Should a copy of the whole sale deed be attached ? It contains a lot of pages.

No. Take a copy from Page 1, until the page where your flat number is attached. The whole document is not required.

I do not have an address proof at this address.

You can attach a relative's address proof in the same address, and a relationship proof.

Eg. 1: Parents staying with children.

Attach a copy of one of the child's address proof, with a copy of the child's passport or birth certificate - which shows parents names.

Eg. 2: Wife has no address proof.

Attach a copy of husband's address proof, along with a copy of wife's passport showing husband's name Or Marriage certificate in English / Kannada.

Eg. 3 : Children staying with parents

Attach a copy of the parents address proof, and the Child's passport or any other document showing parent's names.

Please note: Aadhar card ***cannot*** be used as relationship proof document.

Why is Aadhar not accepted for address proof ?

The election commission has a specific list of documents acceptable as address proof. It has not yet been updated to include Aadhar card. Over 200 applications were actually submitted with Aadhar as address proof in-spite of the checklists and instructions. We are only trying to help. The rules are set by the election commission. Kindly do not argue with us.

I have submitted my form Online in NVSP . Do I need to do anything more ?

Physical copies of the following must be submitted. Or else your application won't be processed.

1. Printout of screenshot of reference ID with online status. Enter the reference ID in NVSP website. Take a print of the page. You can do this from Mobile or computer. Both are accepted.
2. Behind this print, pls write

3. Print the address proof that you uploaded online. [Aadhar is not acceptable as Address proof]

ceokarnataka site doesn't show an error if Aadhar card as address proof. It is a problem in the site and the application will get **REJECTED** if you submit Aadhar card as address proof.

4. Submit the documents to JLL office at Tower 8 basement.

If you do not submit the physical copies, your application will not be approved.

Mobile and E-mail are optional as per the form. Why do you need this ?

We are a small team handling hundreds of applications. When we need to contact you, it is easier if we have email ids or mobile numbers where we can contact you directly.

When election commission guys need to contact you to ask for additional documentation or to give your ID card, they can't call on intercom. They can call only on mobile.

Is this a service from JLL ?

No. This is purely a volunteer driven effort by "Whitefield Rising". Infact, JLL personnel are helping us with collecting documents and liasoning with BBMP etc. Thanks to Mr Devendar and Mr Manjunath.

Why is it repeated so many times that Aadhar card is **not accepted** as address proof ?

More than 200 applications have been submitted with Aadhar card as Address proof.

The ceokarnataka site is wrongly accepting Aadhar card as address proof. It's a website error.
