You can download all forms required by the Patuxent NJROTC program below. Click on the name of the form to print or download the form.

All cadets must have the following forms completed and turned in to NJROTC to participate in NJROTC activities. All forms must be re-done yearly or earlier. 

            1. NJROTC Standard Release Form                  

 2. Cadet Data Form

                  3. Health Risk Screening Questionnaire                  

4. Pre- Participation Physical

             5. Over the Counter Medication Administration 

             6. Prescribed Medication Administration Form (if applicable)
