Go into each patient chart-
Ensure HIPAA, ACC, Financial Consent, and Consent to Treat are completed and in attachments.
When patient has traditional Medicare policy they are required to sign a LBA form.
If the patient was in an auto-related incident, they need to fill an auto disclosure form for each provider they are treating with.
Check for license and insurance card.
Make sure these aren't expired.
Make note in "Appt. Notes" section
example "ALL PPWK DONE- INITIALS" or "SCAN ID & INSURANCE CARD- INITIALS"
If they are getting an injection-
Look for pre-auth in attachments and include how much to collect.
Include this on chart prep note.
Use Appointment Flow and filter to see the patients for your provider(s).
Click on each patients name.
Under Manage Attachment tab verify that the ACC, HIPAA, LBA (for MCR primary patients), AUTO (for Auto primary insurance patients only) are completed.
4. If forms need to be completed by the patient [insert how we will do this with Klara]
5. Once all forms have been sent to patient exit out of the account taking you back to Appt Flow
and under APPT NOTE type in what the patient is needing.
Most commonly:
ID/INS NEEDED, PPWK SENT
ID/INS IN SYSTEM, PPWK COMPLETE
ID/INS IN SYSTEM, SIGNATURES MISSING
Always include your initials after the note.
