Here is the Cause/Fundraiser selection form. Fill this out once your chapter has voted on the cause and fundraiser to support.
Additionally, Operation Smile believes in providing students with recognition for their dedication to the cause. Each meeting a member attends and each fundraiser a member assists with should count toward community service hours, this will be validated by the chapter advisor. Before starting a chapter, talk to whomever is in charge of community service hours at your school and discuss how the implementation of this concept works for you!
