Forms for Students
Application materials should be completed online (email to edulo@vermontstate.edu) by the deadline (February 15 for fall semester practicums, and September 15 for spring semester practicums). To complete an application, click the link below for the appropriate TEW section application and follow the digital prompts.
Forms for TEW2
In the second Teacher Education Workshop (TEW2) you will learn when and how to apply for Practicum I. Complete and submit the linked digital application by February 15 (for fall semester practicum 1) or September 15 (for a spring semester practicum 1).

Once students have received their confirmed placement from VTSU Placement Coordinator, Marni Leikin, they must complete a background check before their placement begins. Please refer to this checklist for instructions:
Forms for Cooperating Teachers
Cooperating Teachers: please download, complete, and submit forms to edulo@vermontstate.edu. Save a copy for your own records, and give one copy to the Teacher Candidate as well.

Practicum I Forms
Submit these forms early in the semester (Sept. 15 for fall, Feb 1 for Spring)
W-9 (complete and submit this form if you have never received payment from NVU before)
Submit these forms later in the semester
Placement Verification Form (at the end of the semester/placement)
Submit this if necessary at any point in the semester
Practicum II Forms
Submit these forms early in the semester (Oct 1 for fall, March 1 for Spring)
W-9 (complete and submit this form if you have never received payment from NVU before)

Practicum II Evaluation Form (Complete this once at the end of the Practicum II semester)

Student Teaching Forms

Request for Payment Form: Student Teaching with 2 Teachers, shared classroom ($250)
W-9 (complete and submit this form if you have never received payment from NVU before)

Student Teacher Evaluation Form (complete once at mid-term and once at the end of the semester; the mid-term evaluation is required to receive payment)

Practicum I Forms
Lesson Observation Form (Use this form when the Teacher Candidate is teaching and you have seen the lesson plan ahead of time)
Informal Observation Form (Use this form when the Teacher Candidate is not lead teaching during your observation)

Student Teaching Forms
Observation Log (Schedule an observation of your student teacher at least once every 10 working days and submit the log after the final meeting)

Informal Observation Form (Use this form when the student teacher is not lead teaching during your observation)
