No medication will be accepted until ALL required paperwork, including Action Plans completed by the physician have been completed and turned into the clinic (see all forms below).
When possible, all medication should be given at home. However, some medication will be dispensed at school according to the following guidelines:
The medication must be in the original container with the pharmacy label and must not be expired.
A physician’s authorization form must be signed by the physician and parent if a medication is to be administered. Directions must include name, date, name of medication, dosage, route of administration, and time to be administered. (See forms below)
Medications must be given the school nurse by a parent or guardian. The child cannot bring in their own medication.
If the medication is new to the student, the first dose of the medication must be given at home.
Over the counter medication such as Tylenol or Ibuprofen must have a label from the pharmacy and a form signed by their physician.
If your child has asthma, an Asthma Action Plan must be filled out by a physician.
If your child has a food or environmental allergy, an Allergy Action must be filled out by a physician. (Note: if student has a food allergy, an action plan AND Dietary form must be filled out by a physician.)
If your child has seizures, a Seizure Action Plan must be filled out by a physician.
Cough drops/creams/ointments that are kept in the clinic also require a parent permission form.
*If your child has suffered an injury or recent surgery requiring restrictions from physical activities (PE or Structured Recess) please locate the Physical Education Restriction form below and have it filled out by a physician.
