Immunizations

a student must be fully immunized against certain diseases or must present a certificate or statement that, for medical reasons or reasons of conscience, including a religious belief, the student will not be immunized. Please see our district handbook for detailed information.

Required Vaccinations For Elementary Schools:

PRE-KINDERGARTEN

DPT, DTAP, DT, Td-Five doses of any combination DTaP/DTP. Four doses are acceptable if fourth dose is on or after 4th birthday.

POLIO (IPV)-Four doses with the last one being after the 4th birthday. Three doses are acceptable if the third dose is on or after the 4th birthday.

MMR-Two doses of MMR vaccine with the first dose on or after the 1st birthday.

HEPATITIS B-Three doses are required.

VARICELLA-Two doses of varicella vaccine on or after 1st birthday.*

HIB-Three doses of HIB ( a primary series with one dose on or after 1st birthday) or 1 HIB on or after 15 months up to 59 months

PNEUMOCOCCAL-Complete series or 1 dose after age 2

HEPATITIS A-Two doses of hepatitis A vaccine on or after 1st birthday.

KINDERGARTEN THROUGH 6th GRADE:
