Thank you for enquiring about enrolment at Newmarket OSHC!

This page guides you through the steps to secure successful enrolment at our OSHC Service.

Please click the Enrol Here button to complete your online enrolment form. As you complete your enrolment, we kindly ask that all forms are completed in full. The only sections that may be left blank are questions relating specifically to early childhood care, such as nappy cream, teething gel, and similar items. 

Please note that providing a copy of your child's birth certificate or Medicare card is not mandatory. However, we do require all families to provide:

• A current copy of your child's immunisation record; and

• Any applicable court/parenting orders and anaphylaxis, allergy or asthma action plans for your child.
