TK/ Kindergarten Checklist

Welcome to Kindergarten!  Before the start of the school year, your child will need the following forms turned into the health office.  

* Health Information Sheet explains the following forms- dark pink

o   Immunization Record Form-example in gold, please turn in completed form from your doctor or immunization clinic to the office. Health information sheet explains the requirement. Documentation of a valid Certificate of Immunization Exemption (Religious or Medical) or Provisional Certificate of Immunization may be submitted in lieu of a Certificate of Immunization.

o   Lead Level Completed * No form needed to turn in, but needs to be done.

o    Dental Screening Form-yellow

o    Vision Screening Form-green

o   School Medical Report (Physical Form) *optional but strongly recommended- blue

Infinite Campus Parent portal Survey-  Health Questionnaire and Consent, will be sent out the week of registration and should be completed before the first day of school.
