Kindergarten Checklist

Welcome to Kindergarten!  Before the start of the school year, your child will need the following forms turned into the health office.  

* Health Information Sheet explains the following forms- dark pink

o  Student Health Information Form-light pink, please turn this form in at information night (March 4)  or ASAP to the school office. 

o   Immunization Record Form-example in gold, please turn in completed form from your doctor or immunization clinic to the office. Health information sheet explains the requirement.

o   Lead Level Completed * No form needed to turn in, but needs to be done.

o    Dental Screening Form-yellow

o   Vision Green Card-small green card 

o    Vision Screening Form-white  

o   School Medical Report (Physical Form) *optional but strongly recommended- blue
