9th Grade Dental
Dental Screening Requirement: All children enrolled in 9th grade are required to have a dental screening pursuant to IDPH 641 Chapter 51. This screening needs to be completed no earlier than 1 year prior to enrollment of 9th grade (after Aug. 24th 2022) and no later than December 1st , 2023. Please turn your completed Dental Screening Form found under health forms on the district website (link below) to your child’s school office or have your dental office fax to the high school office at 712-737-3933.
