Asthma Inhaler Self-Carry Permission Form

For older students that require an inhaler at school for asthma or difficulty breathing, Iowa law allows them to self-carry their medication if they are responsible enough to do so. The above form needs to be filled out with a parents signature along with a physician signature. please turn into the school office.
