ABSENCE FORM

This form is to authorise student absence from school and can be used for absences during part of a day or for a complete school day. We request that you provide your full name and DNI/NIE/official ID number to complete the authorisation.

IMPORTANT NOTE:

If the absence is for part of the school day you should indicate the same DATE for the first and last day of the absence.

If the absence is for one or more full days you should indicate 9 am in the TIME of the first day of absence and 4 pm in the last full day of absence.

Mediante este formulario autorizas la ausencia del colegio de un alumno, tanto de manera parcial (algunas horas durante un día) como días completos. Os pedimos que indiquéis vuestro nombre completo y el DNI/NIE para cumplimentar la autorización.

NOTA IMPORTANTE:

Si la ausencia es parcial (unas horas en un mismo día), os rogamos que indiquéis la misma FECHA para el primer y último día de ausencia.

Si la ausencia es de uno o más días completos, indicad en la HORA las 9:00 horas en el primer día de ausencia y las 16:00 horas en el último día completo de ausencia.
