You must also submit the Licking County Health Dept Temporary Food Application. Forms can be:

Payment can be made over the phone with credit or debit cards by calling 740-349-6475. A $30 fee is required for facilities classified as tax-exempt non-profit [501(C)(3)], while a $60 fee is required for all other facilities.
FIRE DEPARTMENT REQUIREMENTS
Propane tanks must not be within 10 feet of a tent.
Food trucks cannot be within 7 feet of a tent and cannot be within 3 feet of another food truck or trailer. This includes over hangs and awnings
If you are using oil or grease you will need a class K extinguisher
Fryer vent 12 inches or cooking appliance that produces heat may not be within 12 inches of a side wall or divider wall
Every propane must be secured empty or not at all times. Does not matter if it is plugged in or in use. 3 or more can be tied together 2 or less must be staked.

REQUIREMENTS FOR LEGEND VALLEY EVENTS/PRODUCTIONS
FIRE EXTINGUISHERS - OFC SECTION 906
All structures used for the making or selling of food shall have a minimum of 1 (one) tagged, inspected, and
charged 10 lb Class-ABC extinguisher. Any cooking that produces “grease-laden vapors” must have a tagged,
inspected, and charged Class-K extinguisher. Appliances that produce “grease-laden vapors” include, but are
not limited to: flat top griddles and deep fryers.
All other vendors on the property SHALL have at minimum 1 (one) 3 lb Class-ABC extinguisher. The
extinguisher has to be a charged extinguisher that has either been inspected and tagged in the last year, is
stamped on the bottom the current year or a receipt within 12 months of purchase of the event.
Any worker or employee, regardless of vendor, must have of knowledge of the location and use of fire
extinguishers, including what the different types of accessible extinguishers are.
PROPANE TANKS - OFC 2404.16
Propane tanks containing 500 gallons or less must be secured in an upright position. Propane tanks may not be
attached to the tent structure itself. 20 lb tanks may be placed in a milk crate to prevent tipping. There is to be
NO storage of propane tanks under any tent membrane, regardless of the tank or tent size. It is important that
the vendors and producers/venue staff work together to ensure there are enough posts, straps and securing
devices for ALL propane tanks on property and premises.
***Regarding Propane tanks containing 500 Gallons or more, contact a fire official first ***
COMPRESSED GAS
Any compressed gas systems including, but not limited to, beverage dispensing systems must be secured in the
same fashion as propane tanks.
TENTS, CANOPIES, TRAILERS - OFC SECTION 2403 & 2404
Behind all vendors there SHALL be a 15’ fire lane that is clear of any obstructions including, but not limited to,
trailers of any type, vehicles, tie down ropes and storage containers. The fire lane is to be kept clear at all
times. This is to ensure constant access for emergency vehicles.
Tents used for cooking SHALL have an affixed tag or certificate showing fire resistance.
Any geodome structure or tent membranes without the affixed tagged must have a notarized paper stating the
product that has been used to provide fire resistance and SHALL comply with standard and proper testing from
the fire inspector. A copy of the notarized paper must be presented and kept on file with LTFC.
Any combination of tents, together exceeding 700 square feet SHALL require a 15’ fire lane break. Tents or
structures that are open on all sides and have a square footage exceeding 700 must first obtain a permit to be
erected. Tents or structures that are not open on all sides and have a square footage exceeding 400 must first
obtain a permit prior to being erected.
There SHALL be approved “No Smoking” signage posted in all cooking vendors tents, trailers or stands.
THERE SHALL BE NO COOKING UNTIL A FIRE INSPECTION IS PASSED. NO EXCEPTIONS.
MOBILE FOOD UNITS / “FOOD TRUCKS” - OFC SECTION 320
A mobile food unit is classified as a food truck OR a food trailer.
Mobile food units must follow the same classifications for the ABC and class K fire extinguishers as previously
stated. There SHALL be a 3’ fire lane remaining open to access at all times around the food unit from other
food units/tents. There SHALL be at least 10’ fire lane from any structure. There SHALL be a 15’ fire line behind
the mobile unit. There SHALL be at least 1 (one) carbon monoxide detector on the unit. There SHALL be two
means of egress, each separately not being smaller than [5.7] square feet. LP gas, along with any kind of
compressed gas, SHALL be secured and maintained in accordance with applicable specifications, requirements,
and limitations referenced in the Ohio Fire Code. Any unit using LP gas to cook SHALL provide a readily
distinguishable and accessible “EMERGENCY SHUT OFF” exteriorly. All shut off valves SHALL have proper
signage and identification.
A copy of the food truck check list is attached, can be found online or by calling the firehouse.
A utility task vehicle ( UTV )must be provided and reserved for inspectors during the duration of the event. A
golf cart is NOT a UTV. This insures inspectors can get around on the ground in an efficient manner while
completing inspections.
All permits regarding pyro, fire breathing, or special effects SHALL be turned in 30 days prior to the event.
Reinspection may be granted on the same day if the initial inspection is not passed. Reinspections outside the
initial inspection day are not guaranteed. If an inspection is not passed permission to participate as a vendor at
the event will be denied.

Any emergency exits to include gates need to be clearly marked. Any décor cannot obstruct the visibility or use
of the emergency exit.
