LUMBIA SQUAD IN ACTION....
FIRST PHASE (FEBRUARY, 2023)
A core group named the Bantay Presyon Team (BPT) with 10 selected members consisting of 4 barangay health workers, 1 barangay nutrition scholar, 3 barangay kagawads, and 2 hypertensive individuals. A session was done to inform them about their tasks for the upcoming hypertension-related activities and also about their roles and responsibilities in the community
Workshop on proper blood pressure taking was held on February 23, 2023, at Barangay Lumbia, Ipil Covered Court. The training was done together with the Basic Triage Training from our other CHP on Insufficient Barangay Health Emergency Response Capacity. The training was attended by barangay health workers, the barangay nutrition scholar, barangay kagawads, and volunteers. A lecture and checklist were made for the return demonstration of our BPT, gaining an average score of 90.67%.
Mass BP taking was conducted on February 27, 2023 and March 2, 2023 at Barangay Lumbia, Ipil covered court by the AdZU medical students and Barangay Presyon Team. Only 94 residents of the barangay participated in our two days of mass BP taking. A master list with a total of 61 hypertensive individuals was made for monitoring their blood pressure, whether it was controlled or not.
Utilization of NCD Risk Assessment Form
We were able to utilize the risk assessment form to interview all participants and compute for their BMI during the mass BP taking, and counseling was given on the primary prevention of hypertension to those who were not hypertensive and on how to control their blood pressure by way of compliance and other preventive measures to the hypertensive individuals.
Last March 13, 2023, Health advocacy on Hypertension was conducted at Barangay Health Station of Lumbia, Ipil, Zamboanga Sibugay. The team talked about how important it is to monitor their blood pressure especially those who are already hypertensive individuals.
The team have produced a leaflet on hypertension indicating its description, symptoms to watch for as well as preventive measures on how to mitigate the effects of hypertension. It was a productive approach since most of the residents were unaware of the significance of hypertension with regards to their health. This health promotion campaign is one of the strategies of the Team in the desire to control the emerging number of uncontrolled hypertensive individuals in Barangay Lumbia.
House-to-house Visitation
Last February 2023, the team went to four puroks in Barangay Lumbia, Ipil, to conduct a house-to-house visit to check the blood pressure of residents aged 18 and above for early detection of at-risk individuals for hypertension. But our target during that time was to visit the suspected hypertensive individuals identified during our last survey in September 2022. Only 50 individuals had their blood pressure checked during our visit, and among the 48 unconfirmed hypertensives, only 7 were diagnosed and added to the master list for hypertension.
One of the activities to be done in the strategy of establishing a monitoring system is the production and distribution of booklets to all hypertensive individuals for blood pressure monitoring and a record of their treatment regimen. The booklet has a cover page, and inside you can find client information like the name, age, sex, civil status, contact number, and their proper location in Barangay Lumbia. Aside from that, you can also see in the booklet their record of the treatment regimen the client is taking, some information on the classification of hypertension based on JNC 8, and health teachings on the complications and prevention of uncontrolled hypertension. At the back of the booklet, you can find a reminder about medication compliance and regular monitoring of their blood pressure.
These booklets were distributed during our house-to-house visits and mass blood pressure measurements. Before handing the prepared booklets to the hypertensive clients, we briefed them on their contents.
SECOND PHASE (AUGUST, 2023)
Health Education Campaign on Hypertension: Aksyon Kontra High Blood
On March 13, 2023, health advocacy on hypertension was conducted at the barangay health station in Lumbia, Ipil, and Zamboanga Sibugay, together with some hypertensive individuals and core group Bantay Presyon Team members. The team talked about how important it is to monitor their blood pressure, especially for those who are already hypertensive. The team has produced a leaflet on hypertension indicating its description, symptoms to watch for, and preventive measures on how to mitigate the effects of hypertension.
Another session of a health education campaign on hypertension was conducted during the barangay health assembly on August 23, 2023, by the team and Barangay Presyon Team. 61 residents of the barangay participated.
“DASH Diet lang Malakas” a lecture on promotion of good nutrition
To help hypertensive individuals manage and reduce high blood pressure (hypertension) through dietary choices, a lecture on the DASH diet was held on August 20, 2023. This type of diet would help hypertensive individuals and would serve as a lifestyle modification strategy, as dietary changes are a key component of managing hypertension, often in conjunction with other lifestyle changes such as regular exercise and stress reduction.
On the same day, non-hypertensive residents also came, which was beneficial for them because this kind of diet attempts to lower the chance of developing hypertension, particularly in individuals that have a family history or other risk factors for high blood pressure, and it also enhances overall heart condition.
A leaflet for the DASH diet that is translated into Bisaya dialect was given to the residents who attended the lecture that day. It emphasizes foods that are beneficial for the cardiovascular system, reducing the risk of heart disease, stroke, and other related conditions like diabetes.
“Usok Iwasan para-Buhay ay Maingatan” Smoking Awareness Campaign
On August 31, 2023, students from Jesus Lauron Elementary School at Barangay Lumbia, Ipil, Zamboanga Sibugay, particularly Grades 4,5 and 6, were taught about the detrimental health effects of smoking, including lung cancer, heart disease, respiratory issues, and various other smoking-related diseases. It was also emphasized during the lecture the risks and consequences of tobacco use and protecting non-smokers from the harmful effects of secondhand smoke.
The team has produced a leaflet, "Ang lawas sa tawo nga hinabako," that emphasizes the long-term health benefits of quitting, including improved lung function, a reduced risk of cancer, and increased life expectancy.
“Lambo Lumbia tara na’t mag Zumba”
The team, in partnership with the SK council, conducted a weekly physical exercise regimen held at the covered court of Barangay Lumbia, Ipil, to promote physical activity and provide a way to stay active. It was attended by residents of a mix of ages, from young to old, since the Zumba dance does not require any prior dance experience to participate and is accessible to people of various fitness levels. Maintaining an active lifestyle can have long-term health benefits, such as reducing the risk of chronic diseases like diabetes, hypertension, and obesity. Before heading back to Zamboanga City, a USB containing various Zumba videos and music was handed over to the SK chairwoman for them to utilize and sustain the Zumba exercise in the barangay.
House-to-house visitation to identify hypertensive individuals in the barangay
August 2023, the team conducted another house-to-house visit to four barangays to assess the blood pressure of identified hypertensives, whether their blood pressure got controlled or not, and also their adherence to medication, utilizing the Morisky Questionnaire.
Intensifying the skills of BHWs and BPT through a series of mass blood pressure measurements utilizing the NCD risk assessment form of PHIL-PEN
Another mass BP taking was conducted during the health assembly at the barangay. 61 residents of the barangay participated on August 23, 2023, where 4 individuals were referred for diagnosis and management of hypertension, now with an updated list of 65 hypertensive individuals.
On August 17, 2023, 4 pieces of BP apparatus were procured through sponsorship from a private individual, and on August 31, 2023, 1 BP apparatus was turned over to the Barangay Health Station, which is situated on Purok Mauswagon. The procured BP apparatus will support early diagnosis and monitoring of blood pressure. The other 3 sets of BP apparatus will be handed over once BP stations are constructed and available in each purok of Guma, Lubi, and Mahayag, hopefully by our next exposure.
Referral scheme for BHWs and BPT
A referral scheme was created and checked by Dr. Yrl Phares Martin Agbayani, MHO, and BHS-Midwife, and their suggestions were also included to further improve the scheme. The scheme was turned over last August 2023 in the barangay health station, and this would allow the barangay health workers to refer at-risk individuals for further assessment and diagnosis of hypertension.
Spot map for hypertensive individuals
The spot map was created, and the team utilized push pins: red for individuals with uncontrolled hypertension and white for hypertension with controlled blood pressure. Last September 2023, the spot map was turned over in Barangay Health Station to track the locations of hypertensive individuals, on which there are 22 identified hypertensive individuals living on Purok Mauswagon, 23 identified hypertensive individuals living on Purok Mahayag, 12 identified hypertensive individuals living on Purok Lubi, and 8 identified hypertensive individuals living on Purok Guma.
Medication adherence of hypertensive individuals in barangay Lumbia
In August 2023, the team assessed the medication adherence utilizing the Morisky Questionnaire of Medication Adherence; this was done along with the house-to-house visit of an identified hypertensive. The responses to these questions are scored, and the total score provides insights into a patient’s medication adherence behavior. High scores may indicate good adherence, while low scores may suggest poor adherence, which can be a concern in managing hypertension.
