Once your LAVC Application is processed, you will receive an Acceptance Letter email from noreply@email.laccd.edu that will include your LACCD Student ID Number, beginning with 88 or 90. You will need this ID number to complete and submit a K-12 Form. This ID is not the same as the CCC ID in your CCCApply account.
K-12 Forms are most commonly submitted electronically via Dynamic Forms. Below are the instructions for completing and submitting an online K-12 Form.

You can access the K-12 form above or directly from your Acceptance Letter email.
If this is your first time completing a K-12 Form, select "Create New Account" and follow the steps to create an account.
Log into your Dynamic Forms account. You will be directed to the K-12 Form automatically. Please note that the system will allow you 45 minutes to complete the form before logging you out.
Fill out the form accordingly, beginning with your personal information.
You must enter your LACCD Student ID, beginning with 88 or 90, NOT your high school student ID or CCC ID number.
Enter your high school counselor's name and email address.
Enter your parent/guardian's name and email address.
Sign and date the form.
Enter the term for which you are applying and the year:
If your course begins in January or February select SPRING.
If your course begins in June or July select SUMMER.
If your course begins in August, September, or October select FALL.
Enter the course information: name, course number, and units.
Scroll to the bottom of the page and click Submit Form. Do not sign for your parent/guardian or counselor.
Once you sign and submit your K-12 Form, it will automatically be forwarded to your parent/guardian. Once your parent/guardian signs and submits the form, it will be automatically forwarded to your school counselor.
Your parent/guardian will receive an email with instructions to create their own Dynamic Forms account. Your parent/guardian MUST create their own Dynamic Forms account to sign the form. The student CANNOT sign for either the parent or school counselor.
Your K-12 Form will now be forwarded to your parent/guardian and high school counselor at the email addresses you entered.
Your parent/guardian will receive an email with instructions to create their own Dynamic Forms account. Your parent/guardian MUST create their own Dynamic Forms account to sign the form. The student CANNOT sign for either the parent or school counselor.
Once your parent/guardian signs and submits the form, it will be automatically forwarded to your school counselor.
You can check the status of the signatures in your Dynamic Forms account by going to My Forms and clicking Pending/Draft Forms. Select Action and Manage Co-Signers. If needed, you may edit the email addresses or send the form again.
Once the K-12 Form is signed by a parent/guardian and high school counselor, it will be automatically forwarded to Admissions & Records at LAVC to be processed.
A paper K-12 Form may be submitted in place of the electronic Dynamic Form. The paper K-12 is separate from the electronic version. Do not print the electronic Dynamic Form, as it will not be accepted. Students must get approval from their high school counselor to submit a paper version of the K-12 Form. In the COLLEGE ENROLLMENT INFORMATION box, ensure that the correct campus, term, and enrollment status (part-time) are selected.
If your course begins in January or February select SPRING SEMESTER.
If your course begins in June or July select SUMMER SESSION.
If your course begins in August, September, or October select FALL SEMESTER.
Once you have shared your completed paper K-12 Form with your counselor, your counselor will sign, scan, and email the form to LAVC. LAVC cannot receive the form directly from a student.
