Parental Consent Form for Student Participation

All members are require to submit a signed form in order to participate in Sci-NHS and STEAM Night (which is mandatory). Please turn in your forms as soon as possible to Mrs. Tomlin room 577, or her mailbox in the front office.

Science NHS Volunteering Log

Completed Semester Volunteer logs should be submitted by December 20th.

Seniors who do not complete their hours will NOT receive a cord.

Members who do not meet the 8 hour requirement will have to complete 1.5x the amount of missed hours, on top of 8 for next semester.

Members who submit logs late will have 2 extra hours to complete next semester.

Please print this permission slip and have it signed before FRIDAY 2/9!

You can give the permission slip to us at the meeting on 2/2 or turn it into Ms. Tomlin during B lunch or after school!
