1. Proof of ownership ( for verification and approval of attachment ) - bring original copy & secure
photocopy upon signing of contract.
2. Valid Government id of applicant (Bring original & secure photocopy)
3. If process by representative- need to provide Special Power of Atty (Please see sample form)
4. Valid Government id of authorized representative (Bring original & secure photocopy)

1. Notarized leased of contract- bring original & photocopy
2. Notarized Undertaking Form (Please see DLPC form)
3. DTI/ Business permit (photocopy) – if for business purposes
4. Valid id of lessor / lessee (photocopy)

1. Secretary Certificate or Board Resolution ( notarized, to indicate who will process & sign dlpc contract) .
2. Valid Government id of authorized representative (Bring original & secure photocopy)
3. SEC. Registration with Articles ( photocopy) – if no existing Davao Light account yet.

5. BIR 2303 (photocopy)

3. Valid id of brgy capt - usual signatory (photocopy) or valid id of representative
