These are the responsibilities that must be fulfilled to recieve an A in your clinic placement. You are responsible for knowing the information below and can access this page at any time to refresh your memory. Please consult this page before asking me questions on this topic.
Graduate students should arrive at least 30 minutes before the dx appt. to review case history, prepare materials, etc.
Upon arrival for the dx appointment, graduate students are expected to
Obtain basic client information from IMS as possible, including name, date of birth, age, area of concern, patient history,
Locate and bring the appropriate tests and toys to the diagnostic room
Make copies of the parent questionnaire, oral peripheral examination, language sample analysis, and any other appropriate forms included in your diagnostic binder.
Complete the front of the test protocol once the client has checked in (not before). These are important and will cause the dx to be delayed if they are not done.
I should not need to remind you to do these things at each appointment.
Enter a SOAP note in Cerner for each dx appointment. For no-shows, simply write, "Client did not show for appointment."
Diagnostic reports are due within 1 week. Graduate students can work in pairs to write their first report and take turns on subsequent reports. Save two copies of the report under the client's initials (e.g. KM1 and KM2). I will edit version 2 and change the name to KM Evaluation Report when it is completed. When I am able, I will text or email you to let you know that I have completed it.
Once I have edited the report and saved it with Evaluation Report, graduate students will compare their original copy with my edited copy and note the changes. Graduate students need to create and email me a bulleted list of the changes I made and explain why I might have made them. The list should be broken up into the same headings as the report (e.g. Referral Information, Behavioral observations, etc.).
Use the resources in your diagnostic resource binder AND the sample reports in the clinic drive to write the report.
DO NOT COPY AND PASTE FROM A PRIOR REPORT. EVER. It is a violation of the ASHA Code of Ethics and the University of Memphis policy on academic integrity. It may result in a grade of F for your clinic placement and/or more severe disciplinary actions.
PROOFREAD. Failing to proofread is sloppy and not acceptable in professional writing. When you have completed your report, start at the beginning and read through the entire report out-loud or under your breath. Correct the errors you (inevitably) encounter. It is ALWAYS apparent when you don't proofread and, if I believe that you haven't, I will stop editing and send it back for your to proofread. If I have to do this, expect that your clinic grade will be lowered 1/2 grade each time. My editing should be able to focus on the content and organization of the report. You are in graduate school and it is simply unacceptable for me to have to correct spelling, grammar, or punctuation errors.
Return blue folders to the downstairs HIPAA cabinet immediately once the report is completed.

You are expected to bring the binder you made in LLD or Dr. Eichorn's class to diagnostics. Please, also print the documents in this folder (assuming you don't already have them in your binder). You will also need a binder pencil pouch (or other "zipable" container that can be kept in your binder) to hold a set of picture cards and a penlight. The picture cards cards need to be printed one-sided and in color and laminated (you can laminate them at school). We will add documents to this binder throughout the semester should it be necessary. You are expected to bring this binder to every diagnostic appointment. You are responsible for having and using the documents therein. You will also want to save a copy of this spreadsheet on your laptop for language sample analysis (please do not enter anything into this original copy).
